I expressly consent to REPORTING.CZ s.r.o., with its registered office at Ovocný trh 1096/8, Staré Město, 110 00 Prague, ID No.: 24248991, as the Controller, manually processing my personal data, namely my first name, last name, email address and phone number, in connection with the download and use of the “REPORTING.CZ FOR EXCEL” add-in and in connection with the offering of the Controller’s services (in the form of individual communication, not within the meaning of Act No. 480/2004 Coll., on Certain Information Society Services, as amended, effective as of July 1, 2017).

I grant my consent pursuant to Article 6(1)(a) of REGULATION (EU) 2016/679 OF THE EUROPEAN PARLIAMENT AND OF THE COUNCIL of April 27, 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation), and I declare that all data provided by me is true.

I voluntarily consent to the above-mentioned processing of my personal data for a period of 10 years from the date of its provision.

‍I acknowledge that I have the right to withdraw my consent at any time by sending a message from the email address I provided to: info@reporting.cz with the subject line “Delete from records” or “Unsubscribe”.
